Intructions:

1. Enter the sport and team names.
2. List the Captain first with his phone number in the format of Player / Student ID # / Phone #/e-mail
ex. John Doe / 555-55-5555 / x5555/doe@mc.edu
3 . List each other player with only name and Student ID #.
ex. John Doe / 555-55-5555
4 . After you have filled out the form, press the submit button one time.
5. If the captain does not receive a verification e-mail with in 48 hours, please contact me at Walker00@mc.edu
