Accessibility Accommodation Relevant Forms
The student must provide current (within 3 years), written documentation from a medical physician and/or licensed clinician that verifies his or her disability with recommended accommodations.
Documentation Guidelines
- Documentation provided to the Student Accessibility Services is reviewed on a case-by-case basis. It is best for documentation to be current.
- Recommendations by a licensed professional must be reasonable and effective in assisting the student where the student’s major life activity/activities is/are impaired.

The Online Academic Accommodation Request Form must be completed by the student unless unable due to a disability.
A physician's or qualified evaluator's detailed letter on letterhead stating your diagnosis and the specific accommodation(s) needed in relation to that diagnosis must be provided.
- The detailed letter can be uploaded within the "Online Academic Accommodation Request Form", emailed to accessibility@mc.edu, or dropped off at the Accessibility Services office located in the basement of Nelson Hall
- The clinician may upload documentation through the Online Clinician Form or email it to accessibility@mc.edu
The Online Housing Accommodation Request Form must be completed by the student unless unable due to a disability.
A physician's or qualified evaluator's detailed letter on letterhead stating your diagnosis and the specific accommodation(s) needed in relation to that diagnosis must be provided.
- The detailed letter can be uploaded within the "Online Academic Accommodation Request Form", emailed to accessibility@mc.edu, or dropped off at the Accessibility Services office located in the basement of Nelson Hall
- The clinician may upload documentation through the Online Clinician Form or email it to accessibility@mc.edu
- Clinicians would also need to complete the ‘Physician Documentation’ form located within the ‘Housing Accommodation Request Form’. (The clinician may upload the documentation through the Online Clinician Form)
The Online ESA Accommodation Request Form must be completed by the student unless unable due to a disability.
- A physician's or qualified evaluator's detailed letter on letterhead stating your diagnosis and the specific accommodation(s) needed in relation to that diagnosis must be provided.
- The detailed letter can be uploaded within the "Online Academic Accommodation Request Form", emailed to accessibility@mc.edu, or dropped off at the Accessibility Services office located in the basement of Nelson Hall
- The clinician may upload documentation through the Online Clinician Form or email it to accessibility@mc.edu
- Clinicians would also need to complete the ‘Physician Documentation’ form located within the ‘Housing Accommodation Request Form’. (The clinician may upload documentation through the Online Clinician Form or email it to accessibility@mc.edu)

Documentation Release Request
- Student Accessibility Services will not share the student’s documentation to anyone except to the student and/or a designated individual at the student’s request via the Authorization to Release Information form.
- Instructions for the submission of the Authorization to Release Information form will be within the form.
- The student must provide adequate information regarding the reasoning for the request in order to best fulfill the student’s request.
