Chaperone Forms
Every chaperone must fill out the "Chaperone Expectations Agreement" and the "Mississippi College Hazing Policy." Once a chaperone has filled out the forms during the 2022-2023 academic year, they will not have to fill them out again.

New Member Drop Form
By filling out this form you acknowledge that you are dropping from your organization's New Member Process. If this is done before September 25, you are ineligible to go through Recruitment to join another organization at any point in the future.
Hazing Report Form
The purpose of this form is to encourage members of the Mississippi College community to report acts of hazing by an student, organization or MC sponsored group. Reports will be forwarded to the appropriate Mississippi College official or office. Reports can come from any member of the MC campus community.
You are not required to give your name and personal information, but anonymous reports may be harder to investigate.
